Medical Consent

1. Purpose of This Consent

By submitting a health intake through the PrescrYbe Health platform, you confirm that you have read, understood, and voluntarily agree to the terms of this Medical Consent. This consent is required before any provider review can be facilitated through PrescrYbe.

2. Nature of Services

PrescrYbe Health is a technology platform, not a medical provider. Clinical services facilitated through this platform are provided by independent licensed clinical partners, including OpenLoop Health and affiliated professional corporations. These providers are independent professionals — not employees or agents of PrescrYbe Health.

Submitting an intake and providing this consent does not guarantee that you will receive a prescription or that treatment will be approved. All prescribing decisions are made at the sole clinical discretion of the reviewing licensed provider.

3. Telehealth Acknowledgment

You consent to receiving healthcare services through telehealth technology, including asynchronous review of your health intake by a licensed provider. You understand that:

• Telehealth services have inherent limitations compared to in-person care, including limited physical examination capability

• Technical failures such as internet connectivity or device issues may disrupt the delivery of services

• The reviewing provider may determine your condition requires in-person evaluation and may decline to prescribe

• Your electronic health information will be transmitted securely to the reviewing clinical partner in compliance with applicable law

4. Compounded Medications

Where a provider determines treatment is appropriate and issues a prescription for a compounded medication, you acknowledge that:

• Compounded medications are not FDA-approved drug products. They are customized formulations prepared by a licensed compounding pharmacy at the direction of a licensed prescribing provider

• Compounding pharmacies used by PrescrYbe's pharmacy partners operate in FDA-regulated facilities and comply with applicable state and federal compounding laws

• Individual results vary. No specific clinical outcome is guaranteed, including any specific amount of weight loss or other health improvement

• You must disclose all current medications, supplements, allergies, and health conditions to the reviewing provider so that a safe and informed prescribing decision can be made

5. No Emergency Services

PrescrYbe Health and the clinical partners accessible through this platform are not equipped to handle medical emergencies. If you experience a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use this platform during a medical emergency.

6. Accuracy of Health Information

You certify that all information provided in your health intake is truthful, accurate, and complete to the best of your knowledge. You understand that providing false, incomplete, or misleading information may result in clinical harm and may result in termination of your platform access. PrescrYbe and its clinical partners bear no liability for adverse outcomes resulting from inaccurate information you provide.

7. Prescription Dispensing and Shipment

When a prescription is issued by a licensed provider and transmitted to a licensed pharmacy partner, your order is considered dispensed and complete at the time the pharmacy releases the shipment for delivery — not upon arrival at your address. Once dispensed, prescription medications cannot be returned or refunded, in compliance with federal and state pharmacy law.

8. No Doctor-Patient Relationship with PrescrYbe

Your use of PrescrYbe's platform does not establish a doctor-patient relationship between you and PrescrYbe Health. Any clinical relationship formed through this platform exists exclusively between you and the independent licensed provider who reviews your intake.

9. Right to Withdraw Consent

You have the right to withdraw this consent and discontinue services at any time by contacting support@prescrybe.health. Withdrawal does not affect services already rendered or prescription medications already dispensed. Refund eligibility following withdrawal is governed by the PrescrYbe Refund Policy.

10. Acknowledgment

By submitting a health intake through PrescrYbe Health, you confirm that you are 18 years of age or older, that you have read and understood this Medical Consent in full, and that you voluntarily agree to its terms.
